At open enrollment we will provide comprehensive enrollment material. Our Benefit Election and Reimbursement Request Forms have been designed with the employee in mind. These forms will provide us the information we need and at the same time make enrollment and claim filing quick and effortless to the employee.
Claim Processing
USBAflex knows without rapid claims processing the participation levels will drop fast! Participating employees incur expenses and expect to be reimbursed for their qualified eligible expenses promptly.
